HIPAA Notice of Privacy Practices

Serving Youth Ohio is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

Without your written authorization, we may use or disclose PHI for:

• Treatment — coordinating your care with therapists, care coordinators, prescribers, schools, and other providers involved in your treatment.
• Payment — billing and obtaining payment from Medicaid managed care plans, OhioRISE, commercial insurers, or you.
• Health care operations — quality review, clinical supervision, training, accreditation, and administration.
• As required by law — including mandated reporting of suspected abuse or neglect, court orders, and public health reporting.
• To prevent a serious and imminent threat to the health or safety of you or another person.

Most uses and disclosures of psychotherapy notes, any use for marketing, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

You have the right to:

• Inspect and obtain a copy of your health record, in paper or electronic form.
• Request an amendment to information you believe is incorrect or incomplete.
• Request confidential communications at an alternate address or phone number.
• Request restrictions on certain uses and disclosures.
• Receive an accounting of certain disclosures we have made.
• Be notified in the event of a breach of unsecured PHI.
• Obtain a paper copy of this notice at any time.

Parents and guardians generally have the right to access a minor's records, with limited exceptions under Ohio law where a minor may consent to their own care. Clinicians may also use professional judgment to withhold information when disclosure would endanger the minor.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future. The current notice is always posted on this page.
